Registration Instructions: Please use one form per attendee. Indicate the number of persons who will participate and the total dollar amount for each item. Registration materials provided in St. George
will have tickets and / or information for each of the events and activities designated. Registration and activity fees must be returned to the Bar on or before February 15, 2013 to avoid late fees. Reservations will then be accepted on a space available basis only. Fees include MCLE fees, admission to CLE sessions, materials, Opening Reception & refreshment breaks. REGISTRATION CANCELLATION: Only written or e-mail requests for refunds will be accepted. Such requests must be made to Richard Dibblee, richard.dibblee@utahbar.org. The Bar has no desire to retain monies paid by members unable to attend. However, guarantees and other related expenses
make the following refund schedule necessary. All cancellations are subject to a $50 administrative fee. Convention registration cancellations will be accepted until March 1, 5:00 p.m. No refunds will be provided for registration/activities after March 1. Questions?
